"Patient neglect"

My spouse was in the Derby Hospital, WA for almost a week because they were feeling weak and feverish, they are under NDIS, because they have one leg and is in a wheelchair. Recently, I took them out for an important event and then returned them to the hospital, then went home.

Afterwards on the same day my spouse was discharged, which we were not notified, and taken by an aboriginal liaison officer and a nurse to the units where my spouse lives. But my spouse did not have a key to gain entry to the unit, because my adult child had the keys and was at my house and I believe my spouse told the aboriginal liaison officer and the nurse that if they could go to my house to get the keys or just stay with me that night. But as I understand it, they were flatly refused and told they had to get out of the vehicle even though my spouse did not have a place to stay. The next-door neighbour saw them pushing themselves around wondering what to do, so the neighbour said you can’t be outside like this come in here and stay with me tonight. Which my spouse did.

My spouse was lucky because the next-door neighbour was home, and I was not sure when they were getting discharged, plus I recall my adult child gave one of the doctors their phone number to be contacted so we would know when my spouse was discharged but no one rang them. I felt bad, but where is the duty of care for patients to make sure they’re safe.

Dear mineralwe79,

I am very concerned to hear what happened when your spouse was discharged from Derby Hospital and for the understandable distress this caused your spouse, you and your family. I am sincerely sorry that your spouse was left at their unit and unable to get in, which resulted in them being supported and helped by their neighbour.

We know that discharge planning is an integral part of the care we provide along the patient journey and is so important when patients are transitioning from hospital back to home. I am sorry for what appears to be a communication breakdown, which left your spouse without family support at a time when they needed it. I would like to assure you that I have spoken with staff and we will be undertaking a review of our discharge process to ensure that family involvement is maintained.
