"Chronic Conditions Program at AHC"

About: Albany Health Campus / Seniors Health and Community Rehabilitation

My father, who has a late stage MND was admitted to Albany Health Campus. He was diagnosed with heart failure. During this hospital stay we had a meeting with Ryan, a team member of the Chronic Conditions Program. My father is now in permanent residential care and his involvement in the program has now ceased. However, the support we received from Ryan was excellent. Knowing that we had someone we could call with questions or concerns about my father's medical condition was so reassuring and certainly helped relieve some of the stress of being a primary carer. My entire family would like to thank AHC and the professional, caring and knowledgeable team members of the Chronic Conditions Program.
