"Lack of accessible services for inpatient"

About: Royal Perth Hospital / Adult Mental Health Ward 2K Royal Perth Hospital / Dietitian Service

(as the patient),

I was a patient in Ward 2K as a voluntary patient. The overall stay was pleasant and was calming however there were moments that I believe that policies could be improved and staff could be better trained at handling situations instead of making ultimatums or threats.

1. I was approved to leave the ward from my registrar for a specified time and my nurse at that time was aware of the leave. However, I believe that there was a miscommunication during handover and the afternoon nurse called me and seemingly demanded that I return to the ward immediately, after I negotiated and explained that won't be possible as (i.e outside of Perth), we managed to negotiate a return time which is still not possible given the travel time was approximately 1-2 hours. This further caused destabilising qualities and caused me to have anxiety.

2. During my admission, I made it clear that I had dietary requirements and was undergoing treatment for and was also taking medications for it. I requested for a specific diet, however during my stay no meal was provided to me that suited my dietary requirement as they said a dietician was not available and wont be able to see me in time and told me to use my leave to buy my own food. I could not eat anything that contains certain food as it will exacerbate my symptoms. I had to leave continuously to buy or prepare my own meals.

3. On the last day of discharge, I spoke to my registrar and they were happy for me to be discharged and said I could take 6 hours to prepare to leave. That was the plan and due to my autism, I needed time to process and follow that plan. However before the 6 hours was up, I recall a nurse came into my room and interrupted my routine and made threats that if I do not leave immediately, they will have to call security and I responded “why do you need to call security" and I believe they said because I’m not leaving - further to add “I haven't been given any discharge paperwork, my discharge medication or medical certificate, my supporting letter". The patient beside me told me that nobody was admitted for a few days and it was left empty, so I don't know why they were using apparent threats to rush me. - This further caused me to have a panic attack because I haven't arranged for transport to be picked up that time or have anyone to safely bring me home.

I would also like to add that I am a private paying patient, I hope I'm not charged for any meals as I didn't have any meals (except for the first day lunch) and the last day.

I am pleased to read your overall stay on Ward 2K at Royal Perth Hospital (RPH) was pleasant and calming. This will be conveyed to the staff who take pride in their work and appreciate receiving positive words.

I am saddened to hear of the miscommunication that occurred with respect to your approved leave from Ward 2K. I can assure you this feedback has been sent to the relevant manager and will be discussed with staff highlighting how this interaction has negatively impacted you.

Patient’s individual dietary requirements are noted on ward journey boards to make sure they receive the correct meal. Without specific details it is difficult to ascertain why your dietary requirements were not adhered to during your admission on Ward 2K. This certainly does not meet our expected standards or usually practices performed at RPH and am sorry this did not occur. I can assure you private patients are not charged for meals during their admission at RPH.

It is disappointing you felt rushed as you were preparing to be discharged from Ward 2K. Staff are trained to check patients are discharged with all required paperwork, medications etc and ensure they have means of getting home safely. Your mentioned concerns will be escalated to management for their prompt attention. I would like to sincerely apologise for this oversight and for any distress this may have caused as a result.
