"Heart surgery not happening"

About: Fiona Stanley Hospital / Cardiology, CCU and Ward 4D

(as the patient),

3 years ago, I was diagnosed with Serious Aortic Stenosis. This was monitored each 6 months or so. It deteriorated from Serious, to Severe, to Critical. Late last year, it was determined by my Cardiologist that the Aortic Valve needed to be replaced. I was referred to Fiona Stanley Hospital for treatment. I heard nothing until early this year, an appointment was made to meet the Cardio Thoracic Registrar and the Surgeon. The appointment in Perth lasted only a few minutes, when it was determined I didn't have a required Angiogram, therefore they couldn't say whether I needed a bypass at the same time. To go back to my local hospital and get all the required tests done (why didn't the Cardiologist not arrange the required tests?).

I returned to my home, awaited the multiple trips to my local hospital for the five or six scans that were needed. I was being prepped for the Angiogram, feet in the scanner, tourniquet on my arm - ready to be injected with dye. When a senior clinician came running into the scanner room, saying that they wouldn't allow the scan to take place, as I understand it, because this scanner is not compatible for Fiona Stanley Hosp, I will have to go back up there.

This was arranged, but as my 'carer/partner' couldn't accompany me (due to Covid), it would be best for me to fly up, take a taxi from airport to FSH Murdoch. This happened a few months ago, which I ended up paying for because PATS would cover flight etc.

So despite many unsuccessful phone calls I was not given a date for surgery, until a few weeks ago, a Telehealth appointment was arranged and cancelled, re-arranged and cancelled, to be replaced by a series of phone calls, which constituted a Pre-Admission Clinic, one of the staff I spoke to said a date for the operation would be in the following month, I was to arrange accommodation for 3 weeks, and no they couldn't help with that. I was to present at 6:00am on the day or maybe another time if my surgery was 'second shift', they would call a couple days prior to the appointment to confirm.

So, I arranged accom, drove to Perth with my partner a couple of days before the appointment having not yet received 'the call'. Because none of our phone calls were answered or returned, we went along to the Hospital to find out what time I was to present. I recall the receptionist said oh they can't find me in their system. Despite giving them my Patient Number. They gave us some other numbers to call if we hadn't heard anything by that evening. we called many times finally speaking to someone who suggested we go home. We then got a call from the doctor, who suggested there had been a communication breakdown, and that they would endeavor to get me in the following week but there was no guarantee this would happen.

We decided that we didn't want to run the risk of staying in our hired accommodation for another week, on the off chance we may be seen.

Just as well, on our return journey home, we got a call from a staff member stating that I may get to be seen in the following month.

Meanwhile, my aortic stenosis continued to get worse, and, apparently one of my carotid arteries is 100 % blocked, but can't be fixed until my heart valve has been replaced and bypasses done.

Incidentally, all though this whole saga we have offered to pay towards the operation, our offer has always been rejected. We have Health Insurance with NIB, but can't claim because this is a 'pre-existing condition'. I believe not until next year anyway.

Hi Kellie,

Thank you for your response. I did as you suggested and got in touch with Family Liaison. The person I spoke to was most helpful, they looked into my situation and got back to us within the week. Apparently the date we were given for surgery was not the actual date but a 'tentative' date. How stupid of us not to know that! However they still could not offer us another date, tentative or otherwise.

As we live in Denmark and we were told we have to stay close to FSH for two weeks after surgery, we arranged accommodation for a three week period, and drove up in readiness.

Now according to the FSH website, the wait time for "Elective Surgery Cat 1" is 30 days from referral, we have now been waiting over 300 days. Its not me that assessed my condition all those months ago as Cat1 and told my partner "I am a ticking time bomb"....

So please Kellie can you have a word with your Cardio-thoracic team and get me sorted, or if you/they can't do it please let us know. This waiting around is killing me.

I am glad you have been in touch with our Patient and Family Liaison Service. Unfortunately, Cardiothoracic surgery booking dates are tentative due to the unpredictable nature of emergencies that occur under this specialty, however, I have asked the Cardiothoracic team to be clearer in their communications about this. Please be reassured the team are working hard to review patients on the Cardiothoracic waitlist and prioritise the most clinically urgent cases. I would also encourage you to speak with the Cardiothoracic nurse about your specific arrangements due to being located outside the metropolitan area. All patients on the waitlist are provided the nurse’s details, and you can also contact Patient and Family Liaison on 6152 4013 if you are unable to find the number.

Why am I seemingly being denied Treatment?

Further to my saga detailed above, I have been told by staff at Fiona Stanley Hospital that I am now at the top of the 'Elective Surgery Cat1 Cardio-thoracic' waitlist, I have been given numerous dates for treatment, the latest being Nov 7th, which was bumped from late October, from early Sept.

Now I understand I have been bumped again until November 21st (tentative-maybe). It has now been over a year since my Cardiologist referred me for Surgery for replacement of a failing valve.

What have I done that I should seemingly be denied treatment and messed around so badly for so long?

I was told to write to my MP, which I did months ago, didn't even get an acknowledgement.

I am very sorry that your tentative dates for surgery have not yet transitioned to a confirmed date for surgery, and I completely understand your frustration.

As mentioned by my colleague above our Cardiothoracic surgeons are required to perform emergency surgery with minimal notice at times, and this has a flow on effect to planned (elective) surgeries. They work very hard to ensure everyone needing access to surgery is assessed according to urgency, your situation is assessed and reassessed regularly. We sincerely hope we can confirm and complete your surgery soon.

Again, I would encourage you to speak to your Cardiothoracic nurse about your concerns and upcoming surgery / arrangements. They are well placed to help you and support your needs between now and surgery day.
