(as a relative),

On behalf of my father, I would like to take this opportunity to thank the personnel involved in my father's rehabilitation.

In particular Annie, Kellie, Dianne and Ric who over the last nine weeks have shown tremendous support to my father. Their understanding, empathy, professionalism and most importantly, patience is to be commended.

These four people have assisted enormously in helping my family in making all attempts to get Dad back on track. We are forever grateful for their combined effort and will always be so.

My sincerest thanks.

Dear matraville thank - you for taking the time to write to us and share your family's experience with our TACP Team. We are very happy to read that your Father benefited from this highly specialized home rehabilitation team.

The TACP Team are amazing and have helped so may patients and families transition from their acute care admission to their home with ongoing support to achieve their rehabilitation goals in their own home. We regularly receive many care opinions posts acknowledging with gratitude the wonderful work of the TACP team. We will ensure that your kind words words of thanks are shared with Annie, Kellie, Dianne and Ric.

Thank - you for sharing this beautiful story of recovery with us. We hope that your father continues to recover from his illness and has a full health recovery.
