"My observations following regular experiences"

About: Concord Repatriation General Hospital

(as a service user),

I attend a day therapy service at Concord. Some of the nurses I've dealt with are brilliant; skilled, knowledgeable, caring, and generally hard-working. A few, in my opinion, are the epitome of incompetence. It seems they do not follow basic protocols including the following, based on my observations: medicine checks, that is, checking that the medication has not expired and that it is the correct one for the patient; regular complete observations, that is, blood pressure checks, etc; appropriate assessment after administering certain medication including checking side-effects such as nausea, in fact ticking boxes without asking the patient questions about the effects of the medication and therefore falsifying a health record; and also not administering the full medication because it takes too long.

The fact that this constitutes behaviour seemingly exhibited by more than one staff member and that the behaviour has occurred repeatedly indicates, in my opinion, not just a level of incompetence by the relevant staff members that I have seen, but I think a problem with the management of the ward and a cultural problem.
