"Lack of understanding RE: patient in High Care."

About: Royal Perth Hospital / End of Life (Palliative) Care

(as a relative),

Re my relative. 

My partner is Power of Attorney and next of Kin

1.RPH has my partner recorded with the misspelling of their surname. 

2.RPH has the phone number down as our old House phone which was disconnected 6 years ago.

3.RPH has, I feel, ignored the documents supplied by Craig Care Ascot which gives the latest NOK. Power of Attorney etc.

4.RPH have ignored and refused to update records, as I understand it.

5.Craig Care have contacted RPH and it seems they also have refused to disclose my relative's condition.

6.The Other Power of Attorney (joint) my partner's sibling and relative's adult child has also failed in attempts to ascertain the condition of their parent.

Why is it have to be so difficult in the possible last days of my relative's life.

I am very sorry to hear that information pertaining to your relative’s record have not been promptly updated on our records and the distress you must be experiencing considering your relatives illness and prognosis.

I would like to get this remedied as soon as possible. Please ask your partner to contact the Consumer Engagement Unit on 9224 1637 or email RPH.feedback@health.wa.gov.au

My staff will look into this further and ensure the amendments are made and information updated as soon as possible.

We always endeavour to update patient’s details and information in a timely manner, I apologise that this did not happen sooner on this occasion.
