"Emergency care"

About: Kalgoorlie Health Campus / Emergency Department

(as a relative),

A few weeks ago my child ran through a glass door that was old thin glass not safety glass, small cut to finger large cut to leg, I presented to emergency they were amazing. We went on the ward, went to surgery, stitched up both wounds everyone was amazing.

Then I had to attend 10 days later to get stitches removed. The doctor checked the stitches said yes they can come out . Keep my child on antibiotics and also gave me a cream to put on the stitches The nurse 1 that seen us did not seem confident, I asked them if they had taken a lot of stitch’s out as my child panics, they said yes they tried a few tools and i tries to hold my child still while they tried to cut the stitches .

The nurse then proceeded to get me to hold my child and hold their flash light as they had trouble seeing them.  I said I cannot do both we need help. They went and got someone who wrapped my child in a blanket and we did their leg stitches . Then we wrapped my child again to do their finger. As my child kicked they were held down. When we finished there was blood and I looked down and my child's leg had ripped open, more staff had attended due to my their screaming . And I started to cry it was so overwhelming to see my child's leg ripped open.

Nurse 2 suggested something to nurse 1 and nurse 1 spoke down to nurse 2. The doctor never came to check my child's leg again. They told the nurse to put strips on it a silicone dressing with holes and a bandage over the top

The lovely nurse 2 and 3 helped my child get comfortable near the Christmas tree and they sat still. Nurse 2 told nurse 1 where to find items and again I felt nurse 1 was rude and dismissive to nurse 2. Eventually they put the strips etc in my child's leg. My child got a little restless as nurse 1 put the last Bandage on, they slapped it on said all done you can go now and walked into the admin office .

Then I asked nurse 2 about care. They said the silicone can get wet and stay on for 7-10 days. Nurse 1 opened the door popped out a little then went back  shut it again. I went home. I did not feel confident about my child's leg and I cannot see what it looks like, I rang the health hotline and they said the dressing needs to be changed every day so on day 3 I attended the emergency again, the nurse and doctors was lovely and said they would give me a plan  me how to dress it . Let me soak the bandages and strips off as my child is comfortable with me doing so. And they cleaned it up re dressed it and off I went .

Then 24 hours late I have gone to change it and it is red. So I attend emergency again. Doctor 1 spoke to me and took photos of my obits to show doctors 2,  Nurse 1 is present to. They came over when the 2 doctors came to see me. I explained everything, the doctor 2 asked did they not get the surgical team to look at my child's leg when it spilt open. I said no. They said did they not refer you to wound care I also said no. The nurse 1 was also saying bits and pieces .

The Doctor said can I see the wound I said I’ve just dressed it if you let me soak it off I can show you, I am not ripping it off it will up set my child again. Nurse 1 says I have a wipe to help take the dressing off . I said no. They lean forward to touch my child, I put my arm out and say no. The doctors then discuss a few things and I refuse to remove the dressing as I keep getting all this different information.

The nurse 1 then mentions the dressing again. I then ask the doctors for a confidential conversation. And I explain I do not feel comfortable with Nurse 1 treating my child. I felt they were very rude and dismissive to another nurse when they here trying to help my child. I believe they treated my child like **** when they applied the dressing and I do not feel comfortable.

Doctor 1 and 2 listened then they walked off. Doctor 2 told dr 1 to organise the wound clinic and I said can I get antibiotics . Dr 1 said yes. I then heard nurse 1 say to doctor 2 they was like this and like that and the doctor 2 said don’t worry about it . Doctor 1 came back with my script and I said I didn’t mean to upset you guys, they said just leave it or just drop it I can’t remember.

I asked where the wound clinic was they said down the back near our patients and they handed me my script and walked off. S I just got up and left. I feel as though because I complained, they where done speaking with me so they didn’t want to listen to what my child needed they wouldn’t let me soak off their dressing. S I have been told so many different ways to treat my child it is stressing me out. I feel I was not rude when I said I don’t feel comfortable. And I feel I have a right to ask for staff I feel comfortable with and not be disregarded because I spoke up on behalf of my child.

How much does my child have to go through just to get their leg better. Like I said this is my first bad experience and I stood up for my child and I was dismissed. I have photos I want to send in as well .

Dear oveef54

Thank you for taking the time to write to us about your experience for both yourself and your child.

I am glad there were some positive elements of your care, especially as you have described for your initial presentation to the hospital. I am also glad that you felt confident to continue to return to the Emergency Department when you felt your child needed further care. There are many different types and procedures for dressings now, so it is often best to represent for them to be checked, just as the HealthDirect care line advised.

I am sorry that you felt dismissed by some of the staff and that your child’s care was impacted by you advocating for them. This is not acceptable. We are driving a program of education with all of our staff on the importance of patient-centred care. One of the key elements of this is being able to share stories from our consumers perspective, just as you have provided. However, if you would prefer for us to investigate this incident further, then please don’t hesitate to contact Alicia Michalanney our Director of Kalgoorlie Health Campus on 9080 5817 or email Alicia.Michalanney@health.wa.gov.au to provide the necessary further details.

It is always a good chance for me to raise awareness of the Aishwarya’s CARE Call program too. We always ask people to do exactly as you have done, that is, raise their concerns with their treating doctor or ask to speak with another senior clinician. However, if this had not been the case, patients and families can make an Aishwarya’s CARE Call by calling 9080 5781 or using the fixed phone in the ED waiting room. This will ensure you are put through to a senior member of staff who will listen to your concerns and help you or your loved one with accessing the care needed.

Also, importantly, I would like to make sure that your child is now booked in and seeing the team in Outpatients as part of their dressing clinics. If you have any concerns or have not had an appointment made please call the nurse on 9080 5722. Be sure to leave a message if they don’t answer they will call you back.

Kindest regards

Update posted by oveef54 (a relative)

Thankyou so much for the reply. The next day I attended the wound clinic I was referred to. But it didn’t exist for children not until the new year. I was then directed to the children’s ward where I was told they cannot help as my child and I have not been admitted to the ward. Needless to say I started crying as it had been such a long journey to get the help, I explained everything that had happened and has soaked the bandage off my child.

The nurse said they were so sorry they wished they could help me but I have to go back to emergency to get the wound care I need again. I felt very anxious attending. I waited approximately 3 hours, I recall there was only 2 people in the waiting area when I arrived but then the room quickly filled up.

After a 3 hour wait, my child's leg still uncovered and I had no food as I did not plan for a three hour stay. I went home defeated to feed my child and try again later that night I was also going to call aishwarya care as I just felt so left down and concerned for my child. That day I felt a miracle happened the nurse had spoken in depth to the nurse manager and they called me to attend the ward and finally help my child. They soaked it got rid of all the slime on it and cleaned it up.

I then attended the ward every 48 hours for several weeks. I felt the nurses where incredible. I am so grateful I finally had the help I needed! My child's leg is all better but they have some mental trauma, going through surgery, stitches ripping open and the bandages being changed every 48 hours has caused them a lot of distress. They are starting to slowly calm down now.

I have not been able to reply straight away as it was so much to go through and so exhausting for both of us. I really am so disappointed in the original nurse in emergency when they started to say I had been a problem. I really had not I held my self together very well. I would however like this to be looked into, not to get anyone into trouble but for them to understand I believe they cannot treat patients like that especially when they say they do not feel comfortable with a staff member or the care they are receiving.

Thankyou so much for your reply and advice and Thankyou to the nurses I saw on the kids ward they are incredible, warm, welcoming, helpful! In my opinion, they are such a great team! I have also been in other wards at various times and all the other nurses I’ve encountered have been incredible.

Dear oveef54

I am so glad you got the care your child needed, eventually, and when you did the team have been wonderful. If you would like me to follow up and thank the nurse who took on the role of ensuring that your child got referred to the ward for this ongoing care, as well as the team on the ward, please feel free to reach out and let me know your child’s details. Likewise, I could also follow up with the nurse in the Emergency Department (ED) for the initial presentation and remind them how overwhelming it can be for parents to come to ED with a sick or injured child.

I am sorry that sometimes there can be detours and delays to accessing care. We have been working with our team to remind them to keep the lines of communication open with patients and their families. That is, to keep them updated about what is happening, how to access the next services they need and especially if relevant, to talk them through their options for care pathways and what implications each choice may have.

I truly hope you and your child are now healing; both physically and mentally. Having an illness or injury like this is very traumatic, so please keep taking the time to look after each other and recuperate.

Kind regards, Alicia
