Patient was discharged with VRE info sheet. No information was written on the discharge/transfer checklist or Discharge Summary, When we received the patient to respite, I was extremely concerned for other patients and infection control because I work in the medical profession. I phoned the ward the patient was admitted to and the Charge RN advised that the patient had been exposed to a fellow patient in a 4 x bed, same bathroom who was VRE+.
I believe it is not adequate to discharge a patient home without adequate information, ie. in the Discharge Summary with full information, particularly as this patient was being discharged in to respite situation and has regular admissions to another hospital. In my opinion, just receiving an information sheet on VRE is inadequate.
In future cases, could this please be put in the discharge summary and the referring services be notified so we can arrange suitable infection control measures.
