"Eye injection administered in the wrong eye"

About: Sir Charles Gairdner Hospital / Eye Clinic

(as a relative),

My grandparent was booked in for their 6-weekly routine eye injections to their right eye under care of SCGH eye clinic for right retinal bleed. They have been receiving these anti-VEGE injections for about a year. Recently, my grandparent was brought in to their appointment by their adult child. They saw the specialist and received the anaesthetic injection to their right eye. My grandparent was then asked to take a seat in the waiting room to be called for their anti-VEGE injection. A few minutes later, a similar sounding name was called. My grandparent’s child clarified with the nurse whom called the name, if it was my grandparent’s name who was being called and they responded yes to this. My grandparent was brought inside to have the injection administered and their adult child was asked to wait outside.

As the doctor started to review my grandparent's left eye, they said that it was not this eye, it was their right eye. The doctor reviewed the file and insisted that it was the left eye according to the notes and despite my grandparent insisting multiple times that this was not correct, the doctor proceeded with administering the injection to their left eye. This resulted in my grandparent receiving the injection to their incorrect eye. The team later determined that they had selected the wrong file and that the file was corresponding to another patient by a similar name. Despite multiple health care staff present (nurses, doctors), I believe nobody undertook a 3-point ID check, confirmed my grandparent's full name, date of birth and address. I feel this must change for patient safety. My grandparent repeatedly told the doctor that this was not the right eye and yet the injection proceeded to be administered. I feel my grandparent's consent to receiving this injection was not provided given that they were aware it was not for the correct eye, however this was overwritten by the staff. This practice must change. Please contact me for any additional information.
