"My parent's stroke follow up post discharge."

(as a parent/guardian),

My parent had a severe  stoke and was first taken to SCGH  and five days later was admitted to FSH to go to State Rehabilitation  service  for their therapy.  My parent was there for approximately six months and had a few other health issues that required them to go back to the main department. They received excellent care while they were there and I like to thank everyone who cared for my parent. But since discharge I feel like my parent has been forgotten because they were supposed to go back 3 months after discharge but it has been 12 months and they haven't gotten an appointment.

I am so sorry to hear that you parent recently suffered a severe stroke, I can only imagine how distressing and uncertain a time this would have been for you all. Given everything you would all be navigating at the moment I appreciate you taking the time to share your feedback with us on Care Opinion. Platforms such as this provide us with a really valuable opportunity to learn and understand what areas we are doing well in, and where we can improve.

You have touched on a few different issues that are of concern to me, as it certainly does not meet our commitment to providing high quality patient centred care nor is it reflective of our values. I have shared your feedback with the Service Directors who are responsible for managing our State Rehabilitation Service and they are in much agreement with me that this is not the experience we want for our patients.

I would like to be able to speak to the team involved so I can get a better understanding of what occurred and the details around the discharge planning. It sounds as though there are crucial learnings, we can take away from your parent’s experience.
