"Not receiving the correct care"

About: Karratha Health Campus

(as the patient),

I had been recently in and out of hospital all week very unwell, after my second hospital admission I was diagnosed with a pulmonary embolism. Due to a rare kidney condition I waste potassium and magnesium and frequently require potassium transfusions if I’m unwell with other illnesses/disease, once this had been corrected for a second time in hospital that week I had been discharged.

I presented again a few days later stating how unwell I suddenly felt, chest pains, heart palpitations etc and advised the doctor I suffered from a kidney condition that wastes electrolytes. They acknowledged this and began assessments. I had bloods, check over etc done and was advised I was ok to go home everything came back normal and that the chest pains etc how I was feeling was due to the PE and recovery was going to take time.

The next day in the evening I get a call from my local GP as she received the bloods that were done in hospital, she was in a panic that my potassium was dangerously low and I needed to get to the hospital immediately.

My potassium was 2.8 at the time I was discharged from hospital the night before, I feel this is absolutely unacceptable.

In my opinion, if the doctor had of read my clinic notes and previous admissions that week they would have seen that I had this condition and this was the first thing I feel they should have checked especially with how I presented.

I feel I could have gone to sleep that night at home and had a heart attack.

I placed my trust in the care of the doctor to check the appropriate things which I feel could have ended with me losing my life.

At the time of writing this, I since still haven’t received a phone call or any acknowledgement even though I brought this to their attention.

Dear Salsb36,

Thank you for coming forward with your concerns regarding the care and treatment you received at the Karratha Health Campus Emergency Department.

I’m sorry to read of your frequent attendance due to being so unwell and subsequent diagnosis of a pulmonary embolism. I am also concerned to note the care provided by our Emergency Department team was not responsive to either your underlying health condition or your acute hospital presentation. Respectful communication is essential in providing person-centred care and I am truly sorry that you felt unheard and have not received a follow up call since bringing this to the attention of staff.

We would be grateful to learn more about what happened to be able to initiate a thorough review with our clinical team as to your presentation. It would also present an opportunity, and if you were willing, to involve you in discussions after completion of the review of any outcomes and recommendations as a result. I would appreciate an opportunity to speak with you. Please feel free to contact me at your convenience on 08 9144 7652 or via email at: Matthew.Wells@health.wa.gov.au

Again, thank you for taking the time to share your story. I do hope that I hear from you in the near future.

Kind regards

Matt Wells
