"School Entry Health assessment form"

(as a parent/guardian),

On the second week of my child attending kindy we received home the School Entry Health Assessment form. I was appalled to see under the child details section ‘preferred pronouns’. This form is targeted at children from 3 years of age and I find the presence of gender ideology on a form for children absolutely inappropriate. I believe including gender ideology on a form for children as young as 3 years old sends the complete wrong message of what is acceptable to expose our children to from a young age. 

I would urge the department to review this section on the form or to create a separate version for children and adolescents.

Thank you for your feedback. We always appreciate parents taking the time to think about the services we provide to the community and give us feedback.

The School Entry Health Assessment (SEHA) form is designed to give parents the opportunity to tell the community health nurse about their child and is also for parents to provide consent for the nurse to undertake vision, hearing, growth, teeth and developmental assessments. The content of the SEHA form is not discussed with the child, however the results of a child’s developmental assessments are provided to parents.

The ‘preferred pronoun’ section is optional, and it is intended to allow for parents to add information about their child’s gender, should they wish to do so.

We apologise that this was not clear in the form and will take your feedback into account in a review of the form.
