"My parent's Hip Replacement"

(as a relative),

I felt the care supplied to my parent in hospital was fantastic but upon leaving hospital the service provider contracted by WACHS, Gardian Nursing to assist my parent's recovery in the home, failed to show up twice to give supervised shower assistance.

This provider called me (NOK) after discharge and did not announce who they were, I had to ask the co-ordinator that called their name, they still did not state who they represented, I had to ask who contracted them to know it was set up by the hospital, i was not given their name only the person to expect a few days later.

Thank you for your feedback regarding the follow up that you parent received after leaving hospital and the difficulties that you faced.

We have recently introduced this service to assist patients when they are discharged. It involves identification of patients who would be suitable and a discussion with them to seek permission to refer to a private provider. I am sorry that the follow up care that was arranged did not occur as was planned and agreed.

My name is Ceri Elliott, and I am the Director of Nursing and Midwifery at Bunbury Hospital and am responsible for ensuring that these follow up visits occur. I do hope that they are recovering well and getting back to the things that they enjoy doing.
