"Post acute care Geraldton"

On Christmas day I broke my hip. They flew me to Perth and then I came back to Geraldton for rehab care and I was in hospital for 5 weeks and then I went home with the care program. The case manager used to come on a daily basis just about. The physio and the case manager who used to take me to all my medical appointments because I’m in a wheelchair and couldn’t walk and they looked after very well. The physio was very good and the case manager herself was very caring. The physio’s name was Kayla and the case manager was called Grace. They went over and above and so a huge thank you to them and all the other people the team who used to come to take me to medical appointment. I’m a big bloke about 6’4” so pushing me around in a wheelchair wouldn’t have been easy! They were very good, I cant speak highly enough of them
