I am a health provider at another service.

I contacted Swan Hill Primary Health to have a health history and list of current medications for a client of mine.

In the phone, I was not asked who I was, where I was calling from, why I wanted the information or if I had consent from the patient. Nothing!

All information required was given without checking who I was, and what relationship I had to the patient.

Thank you for bringing this matter to our attention. Your concern regarding the privacy and security of patient information is extremely important to us. We deeply regret that on this occasion, the interaction led to the release of information as privileged communication without any checks on identity, and purpose, which were not aligned with our policies.

We take such matters very seriously and are committed to addressing them promptly in line with our protocols. We have immediately highlighted this matter and communicated with all team members of the team for additional self- awareness and vigilance.

We will investigate the matter and ensure all staff are re-trained to handle requests for information within our policy framework aligned with confidentiality and compliance with privacy laws. We are committed to implement additional checks to prevent future occurrences.
