"Neck fracture resulting in hospital admission"

About: Geraldton Hospital / Emergency Department Mount Magnet Health Centre

(as the patient),

Recently I fell on my neck, shoulder and head and presented immediately at one of the remote nursing posts with severe neck pain and a limited range of motion following the injury. I was attended to immediately and staff were very friendly and caring however despite these symptoms, the possibility of a neck fracture was not discussed or considered in the initial assessment. This oversight could have had serious implications for my health and safety.

Following an x-ray confirming I had a fracture in my neck I presented at the emergency department. Again I was admitted very quickly and the nursing staff were friendly and caring. I thankfully suffered no additional injury after my initial fall but I do have some concerns regarding my management after admission.

On admission I was not provided with any guidance on how to care for my injury. This included instructions on movement, positioning, or other critical aspects of managing a neck fracture. I arrived in a trauma collar fitted at the nursing post but this was removed and for a long period of time (just under 24 hours) I had no neck support provided while awaiting results, consultations and for staff to locate the correct brace.

I believe clear instructions at the time of admission are essential to ensure patient safety and prevent risk of further injury. The lack of clear instructions at admission lead to uncertainty as both myself and many of the staff were unclear on how to manage essential aspects of care, such as movement, toileting, transfers and sleeping.

While I am very grateful for the care I received and am recovering from my fracture I do want to highlight that consistent information and instructions regarding the management of a spinal fracture is important.

Dear remotetraveller

Thank you for reaching out to us on Care Opinion.

I was very troubled to read of your experience with our Health Service and am grateful you chose to raise your concerns. From your feedback it is clear you believe we did not provide the comprehensive care we should have and for this I offer our apologies.

I would be grateful if you would consider contacting me so we can discuss your concerns in more detail. I will also be able to ensure the nursing post involved receive your feedback and both sites can be provided with the relevant training and we fully support our staff in providing appropriate care for patients with neck trauma. My name is Dr Margaret Sturdy, I am the Director Medical Services Geraldton Hospital. I can be contacted on 9956 8125 or email wachs-midwestcgu@health.wa.gov.au

We acknowledge that interactions with the health system can be a challenging and stressful time. If at any time you feel that you or a relative are not receiving the care or treatment you need, we have a service at the hospital called Aishwarya’s CARE Call. You may notice posters about Aishwarya’s CARE Call in our Emergency Departments – this is a 3-step process for patients, carers and family members to use if they are worried, or they, or a person they care for, are getting sicker in hospital. In the Midwest, no matter what public hospital you go to this number is 1800 316 729 or you can find out more here at: WA Country Health Service - Aishwarya’s CARE Call

I am pleased to hear that all your interactions with staff were friendly and caring and your kind words will be shared with the hospital management team and the teams acknowledged.

I do hope we hear from you and that you are continuing to recover from your injuries.

Regards

Dr Margaret Sturdy

Dear remotetraveller

I appreciate you raising your concerns on Care Opinion.

Like Dr Sturdy I was also very concerned to read your story and wished to offer my apologies on behalf of Mt Magnet Nursing Post for any shortfall in the care we provided.

I would encourage you to make contact with Dr Sturdy so we can fully investigate this situation. Dr Sturdy can provide me with any additional information related to Mt Magnet and I will ensure we provide full support to the team so they can provide comprehensive care that our community can be proud of. Dear remotetraveller,

Thank you so much for your time to outline your interactions with the Midwest region regarding your spinal fracture in July.

Your comments about amazing Matt at the Yalgoo Nursing Post are echoed throughout the region. He is truly a great asset to the community.

Thank you also for your positive comments about the care and service you received through Royal Perth Hospital. I was relieved to hear that you have recovered well and have now been discharged.

Thank you for outlining the opportunities for improvements, including keeping people informed about timelines for reporting CT scans, knowledge about spinal precautions, the availability of equipment, and the training and skills to fit this appropriately. I will liaise with my colleagues at Geraldton Hospital and Mt Magnet to ensure we enact the lessons learnt from your feedback and that we are fully prepared to manage a similar event in the future.

Thank you again for your time.

Regards

Dr Margaret Sturdy
