In early september 2024 my sister was admitted to banksia lodge as a palliative care patient with terminal liver cancer. As her condition progressed we were asked to nominate a funeral service which we did.
I was concerned that if she died in the night that she would be taken away but was constantly reassured that there was no hurry and we would have time to see her before the funeral service collected her.
She died overnight, we were near by and contacted at 6am and immediately said we would come in to see her only to be told that the funeral service had already collected her . This was very upsetting for us as we had no choice as to whether we wanted to see her or not. We think that we would of liked to of seen her one last time .
We had a verbal apology from the nurse on duty who said it was a missed communication and i was told by the palliative doctor that they were from an acute hospital where they have to move people out very quickly.
We hope you can put some thing in place so that this does not happen to any one else .A note on the bed incase at staff handover this information is not communicated or some other way to stop this happening again .
