For the past 3 months my mum was fortunate enough to receive ongoing care in her home from the community health Transitional Aged care team from the Prince of Wales Hospital Randwick. She was discharged from hospital after breaking her hip.
At 76 years old the recovery is long and painful. The Transitional Aged care team provided an exceptional service helping her transition back into normal everyday life. With multiple visits per week Jess, Kim, Annie and Ally were able to help her navigate stairs, walking independently, showering cooking and leaving the house on her own. They do their job so well and genuinely care for their patients. They are determined to reach the goals set and they do so with so much kindness, patience and empathy. A special shout out to Jess who went above and beyond to ensure our mums health and safety was her number 1 priority. We are extremely grateful to have had such an amazing team looking after mum.
