"Elderly parent's hospital admission"

About: Bunbury Hospital / Medical Ward

(as a relative),

My elderly parent in their 90s,  who lives alone had multiple falls, unable to weight bare. Admitted to medical ward Bunbury Regional Hospital,  my sibling and I who have EPG for our parent. We repeatedly asked that our parent not be discharged until they were back to their pre admission level of mobility and self care as they live alone. My parent also has some cognitive decline and needs a advocate to be with them when dealing with health issues.

They were recently discharged into the care of another family member without any consultation with my sibling or I,  no phone calls re discharge planning ,nothing , as their primary emergency contacts I am shocked and disgusted at the lack of empathy , not to mention the duty of care .  My parent has private cover, another private hospital would not even accept them, they have probably had private cover for at least 70 yrs . What a joke.

I was concerned to read of your experiences at Bunbury Hospital. We clearly missed a number of important opportunities to coordinate our care of your parent with you and your sibling.

We are committed to improving the way we provide care for patients, and whilst we get a lot of things right, we remain invested in understanding from patients and carers where we can do better.

To this end, I would welcome the opportunity to learn more about your experiences so we can investigate and find mechanisms to ensure we get care coordination and planning right - every time.
