"Neurodisability Palliative Care CNC"

I am a NDIS Support Coordinator working for Civic Disability Services. My caseload is primarily based in the Sutherland Shire / St George area. My role is to assist NDIS participants to understand and use their NDIS plans, connect them with NDIS providers, community, mainstream and other government services. I assist participants to pursue their goals, build their capacity, maintain their relationships, live more independently and be included in their community.

I began working with Maria in early 2024, when a participant whom I support was referred to her during an extended stay in Sutherland Hospital.

During my time working with Maria, she has assisted with:

- Connecting me with specialists within the Sutherland Hospital relevant to the NDIS participants whom I support

- Driving collaboration, improving communication and accountability within multidisciplinary teams (both hospital-based and external providers)

- Writing support letters to the NDIA for participants to assist with funding requests

- Providing suggestions / sharing knowledge to improve participants’ quality of life

- Advocating for better short and long-term health and wellbeing outcomes for participants

- Promoting inclusion of patients in community

Maria is always incredibly responsive. Maria champions a person-centered approach; she is compassionate, patient and knowledgeable.
