"Needing another form"

About: PathWest Armadale Collection Centre PathWest Jandakot Collection Centre

(as the patient),

I was asked to do repeating blood test every 3 weeks. The laboratory request form was copied and given to me by Armadale Collection Centre, for regular blood tests. 

I used to go to Jandakot Collection Centre because it is the closest to my place. However, as I did not have enough request forms to continue and the last copy of the request form was damaged, I asked the staff at the Jandakot Collection Centre to make another copy, to enable me to continue before the next specialist doctor visit. They refused to make additional copies of the request form so I had to go to the Armadale Collection Centre with the damaged form to got two more copies of that.

Dear corvushf53,

I am sorry to hear that you experienced this inconvenience whilst attending our Jandakot collection rooms. Under some circumstances Medicare allow photocopies of forms to be used, however the situation varies depending on testing and referral circumstances. As I am unaware of the particulars of your test request, could I please ask that you contact our Specimen Collection Manager, Elaine Toogood on 6457 4514 to provide some specific details? It may be that our processes need standardising across our collections centres, and staff adequately informed of any changes.
