"Sample labelling"

About: PathWest Osborne Park Collection Centre

(as the patient),

As requested by the solitary collector on-site, I provided a urine sample and was advised I could leave.   When seeing my doctor several days later, I was told there were no results as the sample was not labelled and lost. We would have thought procedures were in place to contact someone (either the patient or the doctor) if this happened. It was very inconvenient as had to repeat the process.   Subsequently, I went to a different collection office.

Dear hadarze58,

Thank you for alerting us to this issue with your urine sample. I am sorry to hear you were inconvenienced and needed to provide another sample.

We do have processes in place should a sample be received at the laboratory without adequate identification, and usually, we contact the patient and doctor if another sample is required.
