(as a carer),

Dear CEO at FSH

My family member was admitted to your hospital's gastroenterology ward and ICU a month ago. I can say she received the best care from all your staff: medical, nursing, allied health, clerical. They were all proud and happy to work in FSH and also went the extra mile in making my family member's needs met (including putting her favourite meditation in her room). Sadly, she passed away due to her terminal illness. We, her family, acknowledge everything you all did.

First and foremost please accept my sincerest condolences on the passing of your family member.

I really appreciate the time that you have taken to share your lovely story about the good care your family member received at Fiona Stanley Hospital, during what must be a very difficult time for you.

It makes me proud to know that your family member received the best of care at our Hospital. I have passed your lovely feedback on to the ICU and the gastroenterology department and they were very pleased to read your thanks.
