"Poor communication and discharge planning after c-section"

About: Armadale Hospital / Birth Suite (and Antenatal Assessment Unit) Armadale Hospital / Maud Bellas Maternity Ward & Neonatal Nursery

(as a carer),

My wife had a planned Caesarean section due to complications during pregnancy. We were a bit anxious as it was our first c-section but I must say the midwives and theatre team during our surgery were fantastic. They really made the experience less anxiety-inducing.

Our newborn was taken directly to the neonatal unit for observation as the baby had some breathing difficulties at birth. The newly renovated Neonatal Unit is really something - a quiet, peaceful area for parents and babies to recover. The two staff on duty there were also calm, patient and reassuring.

A few hours later Bub and I were released from the Neonatal Unit to be reunited with my wife on the maternity ward, where my wife was very pleased with the caring nature of the nursing staff.

The following day, the doctor during the ward round were great and answered any questions we had. Two important ones we did have were: when will my wife be discharged (to which the doctor answered would be two to three days later) and will somebody be able to provide a medical certificate for my wife so that I can take carer's leave from work (to which the doctor answered that somebody would be able to do that for us prior to discharge).

The following day things took a bit of a turn. We were unexpectedly told by the nurse that my wife would be discharged that day (a day earlier than the doctor had initially said, and we had not seen a doctor since) and then at discharge we tried to acquire a medical certificate, to which we were told by the midwife that, the doctors don't do that. This was contrary to all previous information we had been given by midwives in the Midwifery Group Practice program and the doctor. This forced us to take my wife, still in pain and recovering from her surgical procedure, to our GP so that we could acquire a medical certificate.

Dear saturnyw45,

Thank you for sharing your story with us, firstly may I say congratulations! The birth of a child is such a precious time in your life and I am really proud of the positive comments you have made on the midwifery and medical compassionate care. Our team strive every day to provide the best possible patient and family experience.

Your story has provided us with valuable information to further improve our communication to ensure we are consistent in our messaging. I can only imagine how anxious you were with your first C- Section and baby having to go into the nursery.

I have spoken with the Midwifery Manager and she asked me to pass on sincere apologies on behalf of the team. Most importantly she wanted me to advise they are currently reviewing how the information is provided in the antenatal period and then how that is communicated during your stay and most importantly at discharge. We are considering options such as leaflets and posters on the wall on discharge planning etc. Also to reassure you, we do provide medical certificates and this was definitely a misunderstanding. The Midwifery Manager has provided feedback to the whole team to ensure everyone is providing consistent advice. We will ensure this information is included in the orientation to the unit for new midwives moving forward.

I am sorry you had to attend your GP in pain as it could have been avoided. Hopefully the above improvements can prevent this happening for new mums in the future. I sincerely thank you for sharing your story as the patient experience is very powerful in driving improvement in our service.

Kind regards

Di Barr
