About: Hospital Admission Risk Program (HARP)

(as the patient),

I would like to thank all the members of the team for their support and encouragement during my attendance at the program.   It has been difficult for me to come to terms with the recent diagnosis of heart failure but with the help of the team and the Hospital Admission Risk Program cardiac nurses I am now feeling confident and have a way forward.   Again many thanks.

Thank you so much for taking the time to share these comments regarding the Heart Failure Program and the efforts of staff. I was so pleased you found the program of benefit and also the values the staff displayed made sure you were able to achieve what you wanted, especially with your new diagnosis of heart failure.

It will give me great pleasure to share your comments both with the staff in this program plus also their manager so they can see what a difference they have made to your health and wellbeing.
