"Neglect"

Discharged after having a heart attack to have teeth removed.

I was told I have been discharged and someone in my family was taking me to a dental appointment. I received a phone call from the hospital several hours later asking where I was because they could not find me. 

My open-heart surgery that was scheduled for that week was cancelled.

The second heart attack occurred while at home.

Over the time of being admitted after the first heart attack, I never had the same doctor assessing the condition.
