"Hospital discharge medications"

About: Fremantle Hospital and Health Service / Geriatric Medicine Department & Wards V5&V6 Fremantle Hospital and Health Service / Rehabilitation in the Home

(as the patient),

When returning home from the hospital, I found the medication regime one of the hardest things to get my head around as I tried to contend with settling in, new walking aids, unpacking, preparing meals, visitors and pets.

The medications were all new to me and I believe I would have found it very useful if the medication came in a blister pack for the first week. As it happened, most of the medications ceased within that week anyway.   Fortunately, my lovely physiotherapist was able to go through them with me the following day. Thanks, Lynsey!

Dear New hip recipient,

Thank you for sharing your experience on your return home following a hospital admission. Medications can be one of the trickiest and also most common difficulties facing patients when they return home. I am glad that Lynsey from Rehabilitation in the Home was able to assist you in managing these on your second day home and made the experience somewhat better.

Kind regards,

Kate Gatti
