About: Hospital Admissions Risk Program - Eastern Health

(as a carer),

What a fantastic program I think this is. My husband has a chronic lung disease and he is visited by 2 nurses from this program, in order to keep him out of hospital - Karen and Sonja.

They are so caring and positive and nothing is ever too much trouble.

They also are very supportive of me and always have our well-being front and centre.

Thank you so much for taking the time to share your positive experience. I was so pleased to read how Karen and Sonja from the Eastern Health Hospital Admission Risk Program (HARP) were supporting you and your husband to remain well at home.
