"Wrong records"

(as the patient),

A few days ago, I was given my discharge summary from a few years ago, which should have been given to me almost 4 years ago. The first thing I noticed was that it had I had chronic alcohol abuse, which was wrong as I only drink once a week. On further inspection I also noticed that it had put me down for cannabis abuse, which I have never touched.

I asked the Doctor to change the cannabis abuse and chronic alcohol abuse and take them off my records to which I was told that they could not do so. I am furious with the wrong label and I would like it taken off for good these allegations.
