(as the patient),

A couple of months ago, I sent an email enquiry to the FOI email address asking for direction on obtaining a copy of relevant records/results for an upcoming specialist appointment. I received a reply that afternoon advising that I needed to provide copies of my ID and that, along with my email, was sufficient information for my records to be released. 

I forwarded copies of both my partners and my ID the next day.

I emailed again over a week later to check that my email that had copies of my ID attached had been received and to ask how long it roughly takes to receive the requested information. 

I have still not received a reply nor copies of my records which I do require for my specialist appointment.
