"Oculo-plastics appointment"

About: Sydney / Sydney Eye Hospital / Outpatient Department (Eyes & General)

(as the patient),

Based on my experience, the hospital gives no medical reports or any paperwork regarding the visit. I believe they had no records of my previous visits to the eye hospital and it made it very tedious and difficult to go back through my own calendar and explain.

I would advise making patients that are seen have a record on file there and relevant paperwork given on discharge.

Thank you for your feedback. I apologise that your visits on this occasion was made tedious through the difficulty of finding your medical records.

I assure you all patients have a medical record (either electronic or paper based) for each visit or admission to the hospital. If attending an outpatient’s clinic, a patient’s referring doctor or optometrist will receive a discharge letter once treatment has been completed. All patients who stay in hospital for care receive the relevant discharge information to assist them with their recovery and follow up care.
