"Delays with triage and diagnosis"

About: Bunbury Hospital / Emergency Department

(as a relative),

My parent and I took my other parent (who is a senior citizen) to hospital with a fever of 39.5, rigours, heart rate up to 155bpm, elevated resp rate and O2 Sats of 91. They were slightly delirious and struggling to walk when a nurse met them at the door with a wheelchair.

We waited in the Triage room where one nurse took my unwell parent's temp, asked their age and left the room. Another nurse came 10 minutes later and triaged my parent. We were there about 20 minutes before even being Triaged.

Despite, as I understand it, meeting sepsis criteria at this point, my parent was given a Triage score of 3, given Panadol and wheelchaired back to the waiting room. At this point, I had to leave and my other parent stayed. Approx 45 min later a Dr came to the waiting room to assess my unwell parent. They were moved immediately to resus and from there to ICU with a diagnosis of sepsis.

My parents don’t have a bad word to say about the caring staff especially Liam in ED. They are more than happy with the care provided by ED and ICU staff but in my opinion, things could have turned out much differently and I’m providing this feedback so hopefully, things improve and there’s not a bad outcome for the next patient.

Dear eaglete73,

Thank you for taking the time to provide feedback regarding your parents’ recent presentation to our emergency department. I am sorry to say that it sounds like our normal sepsis process was not followed on this day and for that I apologise sincerely.

I am pleased to hear that once identified staff leapt into action and the following care met your expectations. I would really like to use your parents’ case as a learning opportunity for staff. If you feel comfortable reaching out, I can be contacted on the number below.

I hope that your parent has since recovered, or is on the mend, and I look forward to maybe speaking to you soon.

All the best,

Amber Hargans
