"SHDH Subacute Experience"

About: Swan Hill District Hospital / Sub-Acute Services

(as the patient),

To whom this may concern

I transferred from another hospital, recovering from a total knee replacement  in Subacute  ward.
I am writing to express my gratitude to some very special people who cared for me daily and throughout the night.
 I have never come across such professionalism, caring, attentive, people with such high interpersonal skills.

Lija - I believe you are a very gifted lady who is not afraid to take on any challenge. I felt Lija  knowledge is evident from the care and practice of medical knowledge she applies to her patient’s, I am grateful  to be under her care, my sincere thanks.

Kaitlin- what an amazing nurse, in my opinion, she is a gifted, caring, hard working lady, I feel I am very fortunate to have been placed in her care and would not be progressing as much as I am, Kaitlin  worked above and beyond in supporting me on my journey to recover,

Kyla - from the moment I was admitted  I was assisted to meet my every need, I felt this was conducted in a professional manner and my introduction medically were addressed. I believe Kyla interpersonal skills and her caring nature were above and beyond.

Annette- my rehabilitation to learn to walk from the start was explained explicitly, I was encouraged daily to try my best and achieve my goals to return home, I felt Annette knowledge and communication skills and her professionalism was out standing,  I believe Annette is  a valuable member to the hospital, I thank her for her hard work with me and dedication to her role.

To Subacute team from the happy, smiling tea ladies and gentlemen your service to me was outstanding daily was second to none.

To the cleaners, Stirling job in my opinion, and I believe they need to be recognised for there hard work and dedication.

To the kitchen staff - outstanding meals. I believe they were nutritious, plentiful and presented well with plenty of flavour. Well done kitchen staff.

To all the night nurses-  your knowledge, communication, interpersonal skills were greatly appreciated in my care, I believe I would not be here if it was not for your passion and courage to progress me and demonstrate their ability to care for people.

In reflection, my stay at Swan Hill Hospital was amazing, the staff I saw all work together as a team. I would like my gratitude to be passed on  to all the people that work  at the Subacute ward and to be highly commended for their tireless  efforts.

Thank you for taking the time to write about your experience in Subacute. We always aim for Connected Care, Best Experience here at SHDH, and I am delighted that you had such a positive experience. You have identified a number of clinicians and team members experienced, and junior alike who have contributed to your recovery, and I support Di's comments above.

Also, I was really keen to explain to you that this morning your story was presented at our Clinical Governance meeting which has Board members, consumers as well as senior staff member as an example of a care journey provided here locally. This committee acknowledged your journey and the wider multi- disciplinary team.
