Central Highlands Rural Health says:
We would like to hear about your recent experience of care.
You can post feedback about any aspect of your care.
In particular, we are keen to hear feedback about your experience as a LGBTIQA+ community member. Did you feel welcome and safe? Is there anything we can do to improve your experience?.
Tell Your Story
What is a story?
Tell us about an experience of health, aged or community care that occurred within the last 3 years.
At Care Opinion, your story is feedback written as a brief description of a personal experience of care with a service provider. It usually includes what happened, what you think was good and/or could have been better and how you felt about that experience. Your story can be about you or somebody close to you.
You can use words, our picture tiles or both to help tell your story.

What will happen to your story?
We will read and moderate your story before publishing it on our website if it meets our moderation policy.
Your personal details are not shared with anyone outside of Care Opinion and you are de-identified to help you remain anonymous. (See our privacy policy).
When your story is published (and if it receives a response), we will email you. We will also let the service provider know about your story and invite them to respond to you.
